Community Adolescent Service Practitioner   – Full Time: Permanent 36 hours per week

Community Adolescent Service - Providing a borough wide service

We are looking for an experienced, proactive practitioner who has the ability to work as part of a team , with families often in times of crisis to add value our team

Introducing ourselves:

The Community Adolescent Service provide support to families, children and young people aged 8-18 years, who are at risk of or on the edge of entering the care system and facilitate safe return home to support their plan.

The team work across the borough using agile working arrangements.

Job requirements:

With support you will work with the Social worker and other professionals undertaking a role within the children’s social care plan. You will work directly with young people and their families addressing issues of behaviour management, their wishes and feelings and relationships with significant others. Assisting them to identify their own strengths and difficulties and work towards family resolutions. Issues addressed within the service can be both parental and young person’s substance misuse, mental health issues, NEET, child exploitation, gun and gang crime and youth homelessness. We also work with family to address social or personal relationship issues and violence within the home.

The team work on a rota over a 7-day week, working flexible hours to meet the needs of the families providing planned work and supporting Sefton Emergency duty team as/when required over the weekend period.

We deliver Triple P both group and 1:1 and Strengthening Families programmes and are looking for a practitioner trained to deliver/co facilitate parenting programmes to add value to our team
