Strategic Support Officer– Independence at Home
Fixed Term – 18 Months

The role will be a time limited role to progress the transformation needed within in the Community Equipment Service, widening the use of the Home Improvement Service and Disabled Facilities Grant, and supporting the Integration Agenda to progress. Social Work and OT teams are undergoing a period of change in culture in terms of delivering a Care Act compliant asset based approach, that is prevention focused, and looks to provide just enough support and/or care to promote independence at home and empower the individual. This change in focus means teams need a shift in the tools and services available to them. The demand management programme is progressing in the council and looks to contain the growth in demand we are currently experiencing within the available budget envelope, for this programme to be successful further work on the available services is needed. This role will support a defined programme of change around independence at home services led by the Director of Adult Social Care and delivered as part of the council’s integration programme. This will include further integration of the community equipment service offer and design, review and redesign of DFG Capital funding to support independence at home within a greater range of individuals, exploring an aligned pathway for telecare, adaptations and equipment, ensuring compliance with legal regulations around the procurement, warranty and servicing of equipment we issue, supporting the OT transformation programme and supporting the emerging Integrated Commissioning work plan.
The role is a Strategic Support Officer (a role well established within the Strategic Support function of the Council) with specific capacity and focus on this area.
The post holder will be required to progress a well-defined transformation plan that puts the Community Equipment service at the heart of supporting Sefton residents to live independently with their needs met and achieving the outcomes that matter the most to them.
The service is fully integrated with Health and the post holder must work across Health and Social Care to deliver the best for the service user.

Job requirements: The successful candidate needs to have a flexible approach and show strong leadership and relationship management skills. This is a busy and often high-pressure environment but the successful candidate needs to be able to always work in strong partnership with our colleagues from health and others to achieve the best outcomes for Sefton people.
You will be required to support organisational change projects, leading on allocated initiatives, across the Council with a view to supporting the successful delivery.
The ideal candidate would have experience of working on service improvement within an OT or Social Care environment, and have an understanding of the area of business.
You would be a key member of the team expected to make a significant contribution to the wider Social Care and Health programmes of Improvement You will need to be able to work within budget limitations. The ideal candidate would possess the ability to work well in a multi-agency approach, you will be working with a wide range of professionals both as part of team and as a self starter that can work well independently.
