An exciting opportunity has arisen in the Adult Social Care community Team South.  We are looking for an experienced Lead Practitioner to join our supportive and forward-looking team, with new developments on the horizon which encourage innovative and creative practice.

We require the successful applicant to have the skills, knowledge and enthusiasm to deliver effective management support while meeting strict and defined targets and timescales. Key duties include managing allocation and duty function for the community team, driving up quality and improved standards, supporting and managing staff to undertake Care Act assessments, person-centred Support plans, outcome-focused risk assessments, Capacity assessments and best interest documents. Work includes multi-agency working, working with commissioning staff to monitor and develop services, Court of Protection work (this list is not exhaustive).
