Adult Social Care

An exciting opportunity has arisen for two Social Worker posts to support the new Self Directed Support team. You must be fully qualified with current HCPC registration. You will support the team by re-assessing the needs of adults in receipt of Direct Payments and Individual Service Funds, working closely with Self Directed Support colleagues who will monitor and audit these client’s accounts.
To undertake this role, you must have an excellent understanding of the Care Act 2014, Continuing Health Care, Mental Capacity Act, Direct Payments and Individual Service Funds and other associated legislation.
We are seeking candidates who are self-motivated, can demonstrate a flexible and positive attitude and have an ability to work from their own initiative with minimal supervision. Excellent communication and negotiation skills are essential in order to manage potential conflict.
You must be able to manage and prioritise a complex case load, demonstrate attention to detail, and be able to use creativity and innovation around support planning.
The posts are fixed term until 31 March 2022 with the possibility of funding being extended beyond this.
